Child Health And Disability Team - Vale of Glamorgan - Family support services
What we do
We are a specialist multi disciplinary team who believe that a child with a disability is a child first and should be encouraged and helped to access the same services and treatments as other children without disabilities.
We also link in with the Specialist Health Visitor and Social Workers based in other teams for children who have additional needs.
Referrals to the Child Health and Disability Team can be made by parents, family members and professionals.
To make sure the services we provide are given to people with the greatest need we use an eligibility criteria.
Who do we support
Our service is aimed at :
Children/young people who are aged between 0 - 18 years and who have permanent and substantial disabilities which have a moderate or serious impact on their day to day functioning due to
*A significant visual impairment.
*A significant hearing impairment.
*A profound communication impairment (including Autism Spectrum Disorder).
*A severe learning disability.
*A significant physical disability
We also support parents / carers of children with a disability.
